SYMPTOMS OF BALANITIS
The most common symptoms of balanitis include:
- Redness and swelling around the head of the penis
- Soreness or itching under the foreskin
- Discharge that may be thick, clumpy, or have an unpleasant odour
- Pain or discomfort when urinating
- A tight foreskin that is difficult to pull back
Symptoms can range from mild irritation to more pronounced swelling and discomfort. In some cases, both the head of the penis and the foreskin become inflamed. This is known as balanoposthitis and only occurs in men who have not been circumcised.
Important: Balanitis symptoms can overlap with other conditions, including some sexually transmitted infections. If you are unsure what is causing your symptoms, it is worth getting a proper assessment rather than assuming the cause.
WHAT CAUSES BALANITIS?
Balanitis can have several different causes, which is why identifying the underlying reason matters before starting treatment.
Fungal infection (candida)
The most common cause. The warm, moist environment under the foreskin allows yeast to overgrow and cause irritation.
Bacterial infection
Certain bacteria on the skin can multiply and cause infection, particularly where hygiene is poor or the foreskin is tight.
Skin irritants
Scented soaps, shower gels, fabric conditioners, spermicides, and latex condoms can all irritate the skin of the glans.
Skin conditions
Psoriasis, eczema, and lichen sclerosus can all affect the penis and contribute to balanitis.
Sexually transmitted infections
STIs such as chlamydia are a less common but possible cause, and worth ruling out.
Diabetes
Poorly controlled blood sugar increases the risk, as sugar in urine can collect under the foreskin and encourage yeast growth.

Fungal Balanitis
Fungal balanitis is treated with antifungal ointments or tablets, or a combination of both.
Bacterial Balanitis
Bacterial balanitis is treated with antibiotic ointments or tablets,or a combination of both.
Irritant-related balanitis
Irritant-related balanitis is managed by identifying and avoiding the trigger, alongside hygiene guidance.
Most cases of balanitis respond well to treatment. Dr. Liam will advise on the appropriate course based on your examination.
WHEN TO SEE A DOCTOR
You should book an appointment if:
- Your symptoms have not improved after a few days of careful cleaning
- You are unsure what is causing your symptoms
- Your symptoms keep coming back
- You have recently had unprotected sex or a new partner
- You have diabetes and are experiencing genital irritation or discharge

Q1. Is balanitis an STI?
Balanitis is not itself a sexually transmitted infection. However, some STIs can cause or contribute to balanitis, so it is worth ruling these out as part of a proper assessment, particularly if you have had unprotected sex or a new partner.
Q2. Can balanitis go away on its own?
Mild cases linked to irritation or poor hygiene can sometimes settle with improved cleaning and by avoiding scented products. However, if the cause is a fungal or bacterial infection, it will not clear up without the appropriate treatment.
Q3. Is balanitis contagious?
Balanitis itself is not contagious. However, if it is caused by a candida (thrush) infection, this can be passed between partners during sex. If this is the case, Dr. Liam will advise accordingly.
Q4. Can someone who has been circumcised get balanitis?
Yes, though it is less common. Balanitis occurs more frequently in uncircumcised men as the foreskin creates a warm, moist environment where bacteria and yeast can thrive. Circumcised men can still develop balanitis, but the risk is lower.
Q5. Can I still have sex if I have balanitis?
It is advisable to avoid sex until your symptoms have fully cleared. If your balanitis is caused by a candida infection, it can be passed to a partner during sex. Sex may also cause further irritation and slow your recovery.
